Time and material ticket template
A useful T&M ticket connects the work performed to the hours, materials, equipment, and field acknowledgment. Capture it while the details are fresh, especially when the final price will be reviewed later.
No signup required. The blank text file can be copied into your document editor. The PDF is a worked example, not a blank form.
The fields a field ticket needs
- Job identification: project, location, ticket number, work date, requested-by name, and related change reference.
- Work performed: a specific description, work area, and reason the work falls outside the planned scope.
- Labor: worker or role, regular and overtime hours where relevant, agreed rate, and line total. Explain the basis rather than mixing units.
- Materials: description, quantity, unit, unit cost, and extended total. Attach receipts or delivery references when needed.
- Equipment: description, hours or days, unit rate, and total; distinguish owned and rented equipment if the contract requires it.
- Photos and acknowledgment: useful before/after evidence, signer name and role, date, and the meaning of the acknowledgment.
Example: document quantities before pricing is final
A plumber is asked to reroute a short pipe run around an unexpected obstruction. Record “reroute around beam at kitchen east wall,” not just “extra plumbing.” List the actual hours, fittings, pipe length, and tools charged. Attach a photo of the obstruction and the completed work.
If unit rates or markup still need office approval, say so clearly. A supervisor can acknowledge observed hours or quantities without approving a final price. Do not convert that acknowledgment into a stronger approval in the PDF.
Check the math and the units
For priced lines, multiply quantity by the agreed unit rate. Add the labor, material, and equipment totals, then show any agreed markup or applicable tax separately. Keep hours, days, feet, and each-item quantities distinct.
For example, 3.5 labor hours × $85 = $297.50; adding $86.40 of materials gives $383.90 before any other charges. This is an illustrative calculation, not a suggested trade rate.
Before leaving the jobsite
- Check the work date, project, and related change number.
- Review hours, quantities, photos, and any exclusions with the field representative.
- Record exactly what was acknowledged, by whom, and when. Leave disputed or unapproved items clearly labeled.
- Preserve the signed copy and send it through the project’s required channel when connected.
- Keep a portable backup. If an error is found later, issue a new revision rather than replacing the signed original.
Does a T&M ticket replace an invoice?
No. It supports the record of field work. Your invoice, change-order request, contract approval, and payment record may be separate documents. BuildSlip prepares signed field records and a concise invoice note; it does not collect payment or automatically issue an accounting invoice.
These examples help organize records; they are not legal advice or a substitute for the approval process in your contract. A field signature does not automatically authorize a contract-price change.
